CERTIFICATION OF JUDGMENT BALANCE. This form is used in an Illinois Circuit Court case to provide an official update on the remaining balance owed on a court judgment. Filed quarterly by the attorney or pro se creditor, the form certifies the amount still due from the debtor as of the last day of the most recent calendar quarter, in accordance with Section 12/808.5 of the Illinois Code of Civil Procedure.
