This form is used by individuals who are not licensed attorneys but are filing a property valuation complaint, either for the first time or due to a change of address associated with their existing code number. This form collects essential contact and identifying information including the filer’s name, firm, address, email, phone number, and any previously assigned Assessor code number. It also applies to attorneys who are not in good standing with the Attorney Registration and Disciplinary Commission (ARDC).
