Description

FORM G - NOTICE OF CONFIDENTIAL INFORMATION WITHIN COURT FILING. This form is used in the Thirteenth Judicial Circuit Court of Hillsborough County, Florida, within the Probate, Guardianship, Mental Health, and Trust Division. This form ensures compliance with Florida Rules of Judicial Administration 2.420(d)(2) by identifying any confidential information included in a court filing related to a Guardian Advocacy case for a person with a developmental disability. The filer must indicate whether the document contains sensitive or protected information, specify the legal provision under which confidentiality applies, and pinpoint the exact location of the confidential content within the filing. Common documents that may contain confidential information include petitions for guardian advocacy, applications for appointment as a guardian advocate or co-guardian advocate, psychological reports, credit reports, and death certificates of the ward’s deceased parent. Once filed, the clerk of court reviews the document to determine if the information qualifies for confidentiality under the stated provisions. If the clerk finds that the information is not facially confidential, the filer is notified in writing within five days, and the records remain confidential for no more than ten days unless a formal motion is filed to request continued confidentiality. www.FormsWorkflow.com
