EXECUTOR QUALIFICATION. This form is used in the Essex County Surrogate’s Court, New Jersey, to swear in an executor or executrix named in a decedent’s will. By signing this form, the executor affirms under oath that the attached document represents the true Last Will and Testament of the deceased, and promises to faithfully carry out its terms — including paying valid debts, distributing legacies, preparing an accurate inventory of assets, and accounting to the court when required.
