LASC PRO 077 - PROBATE COURT APPOINTED COUNSEL'S REPORT FOR DEVELOPMENTALLY DISABLED ADULTS WITH MAJOR OR MILD NEUROCOGNITIVE DISORDER (DEMENTIA) ATTACHMENT. This is a Los Angeles County Superior Court probate attachment used in conservatorship proceedings. This form allows court-appointed counsel to report findings regarding a proposed conservatee diagnosed with major or mild neurocognitive disorder (dementia). Counsel must indicate how the diagnosis was confirmed (e.g., capacity declaration, medical records, physician discussions), assess whether mental deficits under Probate Code § 811 impair the person’s ability to manage personal or financial affairs, and evaluate current placement and medication appropriateness.
