DOL-11B - EMPLOYEE LEASING COMPANY ELECTION TO REPORT LEASED EMPLOYEES. This form is issued by the Georgia Department of Labor (GDOL) for leasing companies that choose to report leased employees as their own for unemployment insurance purposes under the Georgia Employment Security Law (O.C.G.A. § 34-8-172). By submitting this form, the leasing company voluntarily elects to assume full responsibility for unemployment tax reporting and compliance, treating leased workers as direct employees for all UI-related matters. The company must provide its business name, address, requested effective date, and total taxable wages paid during the previous four quarters. If in business for less than one year, it must list the date employment of leased workers began. The election requires the posting of a surety bond, cash deposit, or approved securities as specified on Form DOL-12B, which must be filed with the Commissioner of Labor within 30 days of approval.
