PRO 060 - REQUEST FOR PROBATE HEARING IN A DISTRICT COURT: CONSERVATORSHIP/GUARDIANSHIP. This is a Los Angeles County Superior Court probate form used to request that an initial conservatorship or guardianship hearing be scheduled in a specific district courthouse outside the Central District. This form applies only to non-temporary matters and must be filed with the petition or objection. The requesting party must indicate the desired district (such as Pomona, Pasadena, Van Nuys, Long Beach, Torrance, Norwalk, or Santa Monica) and confirm that the petitioner or conservatee/ward resides in that district.
