PRO 033 - SUPPLEMENT TO PETITION FOR PROBATE RE ISSUE (Items 5a(5) through 5a(8), 5b, and 8 of Petition for Probate). This is a Los Angeles County Superior Court probate form used to provide additional information regarding heirs and next of kin in a pending probate matter. The form requires the petitioner to clarify whether the decedent was survived by natural or adopted children, children adopted by third parties, issue of a predeceased child (such as grandchildren), or stepchildren or foster children who might qualify as heirs. The names, ages, and addresses of all applicable persons must be listed either in the original petition or in an attachment. The supplement must be signed under penalty of perjury.
