PRO 035 - SUPPLEMENT TO PETITION FOR PROBATE RE SPOUSE OR DOMESTIC PARTNER (Items 5a(1) through 5a(4) and Item 8 of Petition for Probate). This is a Los Angeles County Superior Court probate form used to clarify marital or domestic partnership status in a pending Petition for Probate. The petitioner must indicate whether the decedent was survived by a spouse (living or deceased after the decedent), was divorced, widowed, never married, or had a registered domestic partner at the time of death.
