PRO 032 - SUPPLEMENT TO PETITION FOR PROBATE RE HEIRS AND NEXT OF KIN (Items 6 and 8 of Petition for Probate). This is a Los Angeles County Superior Court probate form used to provide additional information regarding a decedent’s heirs and next of kin in connection with Items 6 and 8 of the Petition for Probate. The supplement clarifies whether the decedent was survived by parents, siblings, issue of deceased parents, grandparents, extended relatives, next of kin, or relatives of a predeceased spouse—or if no known next of kin exist. The petitioner must select the first applicable statement and list the names, ages, addresses, and relationship of all relevant individuals either in the original petition or in an attachment.
