PRO 044 - NOTICE OF COMMENCEMENT OF PROCEEDINGS FOR A COURT SUPERVISED TRUST (NO FILING FEE DUE). This is a Los Angeles County Superior Court probate form used to notify the court that a trust has been created or brought under court supervision. This notice identifies the trust name, trustee(s), and the court order establishing or supervising the trust. The trustee must attach either the order creating the trust or an executed copy of the trust instrument (Exhibit A), and indicate whether a bond has been filed (Exhibit B).
