APP-011 - RESPONDENT'S NOTICE ELECTING TO USE AN APPENDIX (Unlimited Civil Case). This form is used to tell the trial court in an unlimited civil case, when another party is appealing, that you want to use an appendix as part of the official record for the Court of Appeal of what happened in the case. Use this form only if you are a respondent in the appeal, the appellant chose to use a clerk’s transcript , and there is not a court decision (order) saying that the appellant does not have to pay a fee for the clerk’s transcript.
