DWC/ WCAB Form 10, ANSWER TO APPLICATION FOR ADJUDICATION OF CLAIM. After a medical claim is submitted, the insurance company determines their financial responsibility for the payment to the provider. This process is referred to as claims adjudication. The insurance company can decide to pay the claim in full, deny the claim, or to reduce the amount paid to the provider. This form is used by an employer or their insurance company to respond to an employee's Application for Adjudication of Claim, which is the first step in initiating a workers' compensation case. The purpose of the Answer to Application for Adjudication of Claim is to provide the employer's response to the employee's claim. The form requires the employer to provide information about the employee, including their name, address, and the date of injury. The employer must also provide their own contact information, the name of their insurance carrier, and any reasons why they believe the claim should be denied or disputed. Once the employer files the Answer to Application for Adjudication of Claim, the case will move forward in the workers' compensation system. The next step in the process is usually a hearing before a workers' compensation judge, who will hear both sides of the case and make a decision about whether the employee's claim is valid and should be compensated.
