PRESENT: HONORABLE __________________________ This matter having come before the court upon the stipulation of the parties hereto, said stipulation being incorporated in the within order, and the court being fully advised in the premises; IT IS HEREBY ORDERED that the plaintiff/defendant, shall have custody/parenting time with the minor child(ren) of the parties as follows: 1. 2. 3. This order is granted without investigation by the Office of the Friend of the Court, based upon the representation of the parties that this change is in the best interests of the minor child(ren). The parties agree that there are no pending abuse, neglect, or other proceedings (or orders) involving the minor children covered by this order. The parties agree that if they have failed to disclose information about other pending or existing cases, this order is null and void. Parenting time shall not be exercised in a country that is not a party to the Hague convention on the civil aspects of international child abduction unless both parents provide the court with written consent to allow a parent to exercise parenting time in such a country. IT IS FURTHER ORDERED that all provisions of the judgment entered in this cause, and as subsequently amended, not in conflict with this order shall remain in full force and effect.
