PROBATE COURT OF LUCAS COUNTY, OHIO JACK R. PUFFENBERGER, JUDGE IN THE MATTER * CASE NO. THE ESTATE OF: * * * JUDGMENT ENTRY AUTHORIZING * INVESTMENT OF UNCLAIMED FUNDS * This matter comes before the Court on the Application of the Attorney / Fiduciary of the aforementioned estate for consideration as to the disposition of unclaimed monies in accordance with the Ohio Revised Code Sections 2113.64 and 2113.65. The Court finds that monies in the sum of $ _____________________, to which ___________________________________________________ is entitled as a ben eficiary of the estate, That the fiduciary / applicant has been unable tocate the beneficiary and that sum lo has been unclaimed. [Check if applicable]  Date of death of decedent was prior to January 1, 2002 and the sum has been unclaimed for more than six (6) months. The Court authorizes investment of said sum by the fiduciary for thbenefit of the bee neficiary in the name of the Judge of the Court for the time being for a period not to exceed:  One year the date of death of decedent is prior to January 1, 2002.  Two years the date of death of decedent is on or after January 1, 2 002. The authorized investment of said sum by the fiduciary / applicant shall be as follows: Pursuant to R.C.2113.65, the person investing the unclaimed money shall file in the Probate Court a memorandum thereof, with the original certificate or evidence of title representing such investment, within fourteen (14) days of this entry.
