AO 392 (Rev. 5/81), APOSTILLE INDEX CARD. This is a form used by the United States federal court system for the purpose of requesting an "apostille" certification for a document. An apostille is a type of certification that verifies the authenticity of a document for use in a foreign country. The Apostille Index Card, or AO 392, is a small index card that contains the following basic information: • Apostille Number • Date Issued • Signature • Certified (Print or Type) • Capacity or Title and/or Seal/Stamp of Issuing Deputy Clerk The Apostille Number and Date Issued are used to track and identify the document, while the Signature and Certified information are used to confirm that the document has been properly authenticated by an authorized individual.
