CCCO 0051 - JUDGMENT ORDER (8001). This form is for a forfeiture case in the Circuit Court of Cook County, Illinois. The form is used when the Plaintiff, in this case, the State's Attorney of Cook County, Illinois, has filed a Verified Complaint seeking to forfeit property that was allegedly used in the commission of a criminal offense. The Defendant is the owner of the property. The form states that the Court has found that all parties with an interest in the property have been properly notified of the proceedings and have failed to appear and/or answer the Complaint. The form also states that the Defendant Property was properly seized while in the possession and control of the Defendant, and that the persons entitled to notice of seizure and forfeiture were properly notified according to the statutes as provided. The form then orders that the Defendant Property is forfeited in accordance with the appropriate statutes (e.g., Controlled Substance Act, Cannabis Control Act, Methamphetamine Control and Community Protection Act, or Money Laundering), and shall be delivered to the Director of the Illinois State Police for disposition according to law. Additionally, any right, title, or interest of those persons found in default in the Defendant Property is hereby terminated. Finally, the form orders that the costs of publication shall be paid to the State's Attorney of Cook County from the money subject to forfeiture.
