The student has failed to attend school as required in RCW 28A.225.005. A drug and alcohol assessment is appropriate to the circumstances and behavior of the child and will help the child's compliance with the mandatory attendance law. A mental health evaluation or other diagnostic evaluation is appropriate to the circumstances and behavior of the child and will help the child's compliance with the mandatory attendance law. There is an immediate health and safety concern or a family conflict that needs mediation such that temporary out-of-home placement is necessary. The parent(s) of the student violated the provisions of RCW 28A.225.010. The petitioning school district has informed the student's parent(s) to analyze the causes of absences and has taken steps to eliminate or reduce the child's absences pursuant to RCW 28A.225.020. This court assumes jurisdiction over the truancy of , student, until his/her 18th birthday. The student is ordered to attend school on a regular basis in the School District as directed by the school district and have no unexcused absences or tardies. The student shall complete a drug and alcohol assessment, follow all recommendations in the assessment and abstain from unlawful use of controlled substances or alcohol. The student shall complete a mental health evaluation or other diagnostic evaluation and follow all recommendations. The student shall be temporarily placed in a crisis residential or HOPE center. The parent(s) are ordered to use reasonable diligence to ensure that the respondent student attends school as required by this order. The school district shall file with the court a written progress report by ___________________ (date within 3 months), and every ______ month(s) from the date of the first report. The report must include the following: 1) any additional unexcused absences by the youth; 2) actions taken by the school district; 3) the outcome of the actions; and 4) an update on the youth's academic status.
