That a warrant ordering the arrest of the defendant was issued by a Circuit Court Judge presiding in the County of ___________________________________________ located in the State of Illinois. 2. That the defendant is charged with the offense of _____________________________________________________ in the aforementioned county. IT IS HEREBY ORDERED that: 1. The defendant is remanded to the custody of the Cook County Department of Corrections. 2.
