CHAPTER 11 DEBTOR'S CERTIFICATION OF ELIGIBILITY FOR DISCHARGE THIS REQUEST MAY BE GRANTED UNLESS A PARTY IN INTEREST FILES A RESPONSE OR OBJECTION TO THE CONTENT ALLEGED IN THE CERTIFICATION WITHIN TWENTY-ONE (21) DAYS OF SERVICE. The debtor(s)* in the above captioned matter certifies as follows: 1. 2. All plan payments have been completed and the debtor is entitled to a discharge. An approved provider has notified the court that the debtor has completed a course concerning personal financial management or the debtor has filed Official Form 423 indicating either completion of a course or that the debtor is exempt from the course requirement. The debtor has not been required by a judicial or administrative order or by statute to pay any domestic support obligation as defined in 11 U.S.C. §101(14A); Or B.
