All documents must be on 8½ x 11 inch paper; the Court will not accept other paper sizes. Any false statements of a material fact may serve as the basis for prosecution and conviction for perjury. (2) All questions must be answered concisely in the proper space on the form. (3) Movant seeking leave to file a second or successive petition is required to use this form.
