That I have examined the Clerk's file, docket, and computer record maintained in this matter on _________________ and found that there is proof of service of process on the Respondent by personal service on _________________ substitute service on ________________ or publication on _________________ with the notice required by 735 ILCS 5/2-206 having been mailed. 2. That at least thirty (30) days have elapsed since service of summons or first publication and no appearance has been filed or an appearance has been made but no response has been filed and notice of this motion has been served on the Respondent. 3. That in the case of personal service or substitute service I have given notice to the Respondent of this motion. 4. I further certify that I am prepared to present to the Court the following documents on the date of the hearing of this motion: a. A copy of the appropriate Petition and evidence that all court fees have been paid; b. A copy of this Motion for Default, and notice of Motion as required; c. A completed Affidavit regarding Respondent's Military Service as required by 50 U.S.C. 521; d. A proposed Judgment and, where an appearance has been filed, any Marital/Civil Union Settlement Agreement and/or Joint Parenting Agreement previously executed by the parties which may be appended; and e. In case of personal service, an immediate Uniform Order for Support and a Notice To Withhold Income for Support. WHEREFORE, the undersigned prays that the Court hold the Respondent in Default pursuant to 735 ILCS 5/2-1301, and find that the allegations of the Petition herein are confessed against the Respondent, and set the matter for prove-up immediately.
