Use an additional sheet, or PC-180, if more space is needed. Petitioner (Name, address and telephone number) Relationship to Decedent Bank or financial institution holding safe deposit box (Name and/or branch and address) The etitioner represents that: I am the spouse or next of kin of the decedent . I am a person not related to the decedent but having an interest in the presence of a will or cemetery deed . (Explain the basis for the interest.) I have reason to believe the decedent was the sole owner of a safe deposit box held by the bank or financial institution listed above, which may contain the decedent222s will and/or cemetery deed. WHEREFORE the petitioner requests that the court issue an order to open the decedent's safe deposit box in the presence of an officer of the bank for the purpose of removing the will and/ or cemetery deed from the box.
