IRREVOCABLE APPOINTMENT OF AGENT FOR SERVICE OF PROCESS, PLEADINGS AND OTHER PAPERS BY PARTNERSHIP NON-RESIDENT BROKER OR DEALER THIS FORM SHALL BE FILED IN DUPLICATE ORIGINAL 1.
