An offense or attempted offense under the Sex Offender Registration Act, 730 ILCS 150/1 et. seq. ii. An offense or attempted offense under the Arsonist Registration Act, 730 ILCS 148/1 et. seq. iii. An offense or attempted offense under the Child Murderer and Violent Offender Against Youth Registration Act, 730 ILCS 154/1 et. seq. iv. An offense or attempted offense of a Class X felony v. An offense or attempted offense of aggravated driving under the influence of alcohol, other drugs or intoxicating compounds vi. An offense or attempted offense of aggravated domestic battery vii. An offense or attempted offense of forcible felony (first or second degree murder, aggravated arson, arson, aggravated kidnapping, kidnapping, aggravated battery that resulted in great bodily harm or permanent disability and any other felony that involved the use of physical force or violence against any individual that resulted in great bodily harm or permanent disability). 2. Petitioner has conducted him/herself in a manner warranting the issue of the Certificate for the following time period: a. b. 3.
