IT IS ORDERED as follows: The cases captioned (list all cases here) shall be consolidated under the following caption: 1 Consolidated Ord (rev 6/1/12) UNITED STATES BANKRUPTCY COURT Southern District of Indiana In re: (Case name for lead case), Debtor(s). ) ) ) ) Case No. (xx-xxxxx) All papers, including claims, shall be captioned under the lead case name and number. A member debtor's name should not be included in the caption unless that debtor is an individual. A single case docket shall be maintained after the entry of the order for consolidation under the case number of the lead case. The docket shall be maintained in the lead case only. Once an order has been entered, the member case will no longer be accessible for filings. If consolidation is terminated, documents filed after the order terminating consolidation shall be filed and docketed in the separate cases. After the entry of the order for consolidation, all claims should be filed in the lead case. Claims filed prior to the consolidation order shall be treated the same as claims filed after the consolidation order but shall remain on the claims register of the case(s) where the claim(s) was/were originally filed. ### 2
