CCCO 0023 - NOTICE TO PUTATIVE FATHER (2871) 750 ILCS 50/12A. This form is from the Circuit Court of Cook County, Illinois informing the named father that he has been identified as the father of a child who has been born or is expected to be born soon, and that the mother intends to place the child for adoption. The form advises the named father that he has legal rights with respect to the child, including the right to notice of the filing of the adoption proceedings. To retain his legal rights with respect to the child, the named father must file a Declaration of Paternity with the Clerk of the Circuit Court of Cook County within 30 days of receipt of this notice. The form also advises the named father that he must initiate action to establish paternity pursuant to the Parentage Act of 1984 within 30 days of receiving this notice if the child is not yet born or within 30 days after the birth of the child. If the named father does not file a Declaration of Paternity and request for notice, and does not initiate action to establish paternity as required, then his legal rights with respect to the child, including the right to notice of any future proceedings for the adoption of the child, may be terminated without any further notice. The form also advises the named father that if he denies that he is the father of the child, he may file a Denial of Paternity with the Clerk of the Court, which will be noted in the Clerk’s file, and he will receive no further notice with respect to the child. www.FormsWorkflow.com
