Form CONV LLC-PC has been created for ease in filing, however, you can compose your own document,provided it meets statutory requirements.This form only can be used for a California limited liability company converting to a California professionalcorporation by a majority vote of the members.To file this Form CONV LLC-PC, the California limited liability company must be active on the records of theCalifornia Secretary of State.
