INTERSTATE COMPACT ON JUVENILES REQUISITION FOR RUNAWAY JUVENILE FORM I FORM I TO: (Court or Executive Authority in Holding State) DATE: FROM: (Requisitioning Court or Agency in Demanding State) This court hereby requisitions the return of (Name of Juvenile) in accordance with the provisions of the Interstate Compact on Juveniles. (Additional information to be attached if available.) On the basis of the evidence before it, this court finds said juvenile to be Race: Sex: years of age and to be of the following description: Ht.: Wt.: Eyes: Hair: DOB: Other distinguishing features: Juvenile is believed to be in your jurisdiction at: (Address) This court further finds said juvenile should legally be in the custody of is/are (Parent, Guardian or Agency) who and who is/are located at (Address) within the territorial jurisdiction of this court; that said juvenile has run away; and that said juvenile's continued absence from legal custody and control is detrimental to the best interest of said juvenile and the public.
