McCOLLUM, JUDGE IN THE MATTER OF______________________________________, MINOR CASE NO. ____________________________ ACKNOWLEDGMENT OF RESPONSIBILITY The undersigned parent(s) of the above minor acknowledge that they participated in the application to dispense with the appointment of a guardian of the estate of their minor child. The undersigned further acknowledge that monies belonging to the minor child have been placed into a controlled account in the name of the minor at a depository authorized to received fiduciary funds and that these funds are to be held for the benefit of the minor until the minor reaches the age of eighteen (18) years.
