District of Columbia Government Corporations Division Articles of Merger of Domestic Non-Profit Corporation Form DNP-4. Version 2, July 2010. Pursuant to the provisions of the District of Columbia Non-profit Corporation Act, the undersigned corporations adopt the following Articles of Merger for the purpose of merging them into one of such corporations: 1. Name of each corporation that is the part to merger. NAME OF CORPORATION 2. The name of the surviving corporation. NAME OF CORPORATION 3. The following Plan of Merger was approved by each of the undersigned corporation. (Attach Plan of Merger) 4. The Plan of Merger was adopted in the following manner for each undersigned corporation. Select A, B or C, but not both for Domestic Non-Profit Corporation. A. "The Plan of Merger was adopted at a meeting of its members held on the following date at which a quorum was present, and the Plan of Merger received at least two-thirds of the votes which members present or represented by proxy at such meeting were entitled to cast." B "The Plan of Merger was adopted by consent in writing signed by all members entitled to vote in respect thereof." C "The Plan of Merger was adopted at a meeting of the Board of Directors held on the following date and received the vote of a majority of the Directors in office, there being no members having voting rights in respect thereof."
