Entity Name 2. Cooperative Association Designation elected in the District (Add Cooperative Association or Cooperative if this designation is not required by the state of incorporation) 3. Incorporated under the laws of which state or country 4. Date of incorporation 5. Term of Existence 6. Date you started or will start transacting business in the District Columbia 7. Address of cooperative association in state where it is formed. (If the principal address is outside the state of formation, give the RA in the state of formation.) 8. Briefly describe the proposed activity the corporation will transact in the District of Columbia. ("General purpose" is not an acceptable description) 9. List all officers and directors of cooperative association (attach list if you need more space). TITLE NAME ADDRESS 10. Attach your Certified Copy of Articles of Incorporation and any Amendments, from the State/Country of Incorporation.
