Did you sign it? ____ Copies of the Past Two Years Federal and State Tax Returns with all schedules and attachments. (Including W-2 forms, 1099's) ____ A credit report issued within the past year. One credit report per year is available free of charge at www.annualcreditreport.com. ____ Copies of each applicant's last two pay stubs or other evidence of income. ____ If applicable, a copy of an official statement of social security or other government benefits received by each applicant. ____ If applicable, a copy of each applicant's bankruptcy discharge. ____ List of all monthly income and monthly living expenses. ____ List of each applicant's assets. ____ Do you own a home?
