EXHIBIT & WITNESS COVER SHEET INSTRUCTIONS TO ATTORNEYS: This is the cover sheet for the exhibits which you intend to offer into evidence, i.e., medical reports, medical records, wage records, etc. and witnesses you plan to present. Medical reports should be arranged in chronological order (by date of report, not examination, if different) with the most current report on top. Please tab the most current P&S report for each medical specialty. The tab should be placed on the page which discusses the permanent disability factors. Any transmittal letters should be placed behind the reports. Extra copies of the reports should be discarded.
