RENEWAL INSTRUCTIONS · You may renew no earlier than 60 days before your due date (licensee's birthday). · Pay your renewal fee online with your credit card (preferred and faster processing) or by mail with a check or money order and this form. Online submission of continuing education is not yet available. o No Partial Payments will be accepted. Please pay the amount indicated on this notice according to your Three-Year-Renewal schedule. [Per O.A.C. section 1301:5-1-22] · Complete the continuing education compliance section of this form (pages 3 and 4), attach the proof of completion certificates and submit the package to the division. o If this is your first renewal, your continuing education is not due this year.
