The account shall be opened on behalf of the Minor/Protected Person. The account shall be opened using the sample title, "____ (Name of Fiduciary) as Conservator for _____ (Name Protected Person)". The Fiduciary shall deposit $ _______________________ and funds received subsequently into the account. This person may make internal transfers of funds in order to take advantage of changes in interest rates. It is Ordered that, except for internal transfers, the financial institution shall permit no withdrawals from the account(s), except by separate certified Order of this Court. It is further Ordered that an Acknowledgment of Deposit of Funds to Restricted Account (JDF 867) must be returned to the Court within 30 days.
