CIRCUIT COURT OF CLAY COUNTY, MISSOURI PROBATE DIVISION IN THE MATTER OF ________________________________, WARD CV_________________ ORDER AUTHORIZING ADMISSION OF WARD Now on this date, the Court takes up for disposition the guardian's petition to admit ward to a facility, filed __________________________, and finds that good cause has been shown as to the probability that the ward requires psychiatric care. It is therefore, ORDERED, ADJUDGED AND DECREED that: 1. The above named ward be admitted to ________________________________ _________________________________________________, for the purpose of psychiatric care, treatment and /or placement. 2. That said facility immediately report the date of admission of the ward to the court and to the guardian. 3.
