Indiana Department of Revenue CERTIFICATION OF AIRCRAFT REGISTRATION NAME CHANGE State Form 7548 (R4 / 9-10) This form is to be used to change the name on aircraft registration and to determine state tax liabilities or exemptions INSTRUCTIONS: 1. IN ALL SITUATIONS, YOU MUST COMPLETE THE AFFIDAVIT BELOW. 2. IfyouareaCORPORATIONandyouare: a. FORMING A CORPORATION,attachtothisformacertifiedcopyofyourcorporateminutes(signedbyacorporateofficerand notarized)provingtheaircraftinquestiontobepartoftheoriginalcontributedcapital. b. CHANGING YOUR CORPORATE NAME,attachtothisformacertifiedcopyofyourcorporateminutes(signedbyacorporate officerandnotarized)detailingthespecificsofthenamechange. c. MERGING twoormoreCorporations,attachtothisformacertifiedcopyofyourcorporateminutes(signedbyacorporateofficer andnotarized)detailingtheBoardofDirector'svotetoacceptthemerger. d. WHOLLY-OWNED SUBSIDIARY formerlytitledbyaparentcorporation,attachtothisformacopyofyourcorporateminutes (signedbyacorporateofficerandnotarized)thatclearlyindicatestheownershipofthesubsidiarycorporation. NOTE: All cases of CORPORATE DISSOLUTION (wherby a corporation's aircraft registration has been assigned to a private individual) are considered to be a change of ownership, Indiana Sales (use) tax payment is due upon the transaction. Furthermore, the individual may neither use the corporation's registration certificate nor benefit from any previous excise tax payment by the corporation. 3.
