Please take a few minutes to complete this survey regarding your experience with Mediation in this case. We are sending this survey to attorneys and parties on all sides. Confidentiality The completed surveys are available only to the Court's ADR Program staff. Your responses will be used, in combination with the responses of others, to generate information concerning the value of the Court Mediation Panel and how it can be improved. We would like to provide feedback to the mediators on the Court Panel. If you prefer that your responses not be shared with the Mediator, please check the box to the right.
