INSTRUCTIONS (1) Prepare application and mail to the T.A.B.C., PO BOX 13127, Austin, TX 78711-3127. Multiple sizes may be requested on one application ONLY for products that are IDENTICAL. A set of labels is required for each size. (2) A copy of the Federal Label Approval issued by the Department of Treasury must accompany this application. (3) The applicant must be "The Primary American Source of Supply" within the meaning of Section 37.10 of the Texas Alcoholic Beverage Code for the brand of distilled spirit covered by this application. (Attach a Letter of Authority from brand owner or producer.) (4) Submit one (1) sample of the product for which label approval is desired. If more than one size is requested on this application, submit one (1) sample of the largest size. (5) A fee of $25.00 (twenty-five dollars) is required for each size requested on this application. AFFIRMATION, Under penalty of perjury, I swear I am an officer or authorized representative of the above permittee, and I have examined the information stated on this application, and confirm it is true, correct, and complete.
