C-270w (3/2006) TEXAS ALCOHOLIC BEVERAGE COMMISSION APPLICATION FOR APPROVAL OF LABELS FOR WINE Date: TRADE NAME: PERMIT NO.: MAILING ADDRESS: PHONE NUMBER: Description of Label(s): Brand Name: Class or Type: Appellation and Vintage: Alcohol Content: Size(s) of Container(s): E-MAIL ADDRESS: State here any written, printed, or other matter appearing elsewhere on the container other than on the labels (including net contents if marked on container). INSTRUCTIONS (1) Prepare application and mail to the T.A.B.C., PO BOX 13127, Austin, TX 78711-3127. Multiple sizes may be requested on one application ONLY for products that are IDENTICAL. A set of labels is required for each size. (2) A copy of the Federal Label Approval issued by the Department of Treasury must accompany this application. Note: Samples of wine are required if alcohol content is below 7% by volume and no Federal Label Approval is available. (3) The applicant must be "The Primary American Source of Supply" within the meaning of Section of 37.10 of the Texas Alcoholic Beverage Code for the brand of wine covered by this application. (Attach a letter of authority from brand owner or producer.) (4) A change in Vintage Date does not require a new certificate for a label that was previously approved within the last five years. (5) A fee of $25.00 (twenty-five dollars) is required for each size requested on this application. AFFIRMATION, Under penalty of perjury, I swear I am an officer or authorized representative of the above permittee, and I have examined the information stated on this application, and confirm it is true, correct, and complete. By set of labels is meant all brand, strip, neck, and back labels appearing on any individual container. An exact color reproduction will be accepted if an original label is not available. Since a certificate will be issued for each size, we must have a set of labels for each size.
