LIMITED PARTNERSHIP QUESTIONNAIRE FORM INSTRUCTIONS Authority to do Business in Oregon x Your Limited Partnership must be approved by the Oregon Secretary of State's office to do business in Oregon. List Partners x x x x x List all General Partners. If a legal entity, like a corporation or limited liability company (LLC), owns or controls 10% or more of a General Partner that legal entity must also complete its own separate questionnaire. If there are 20 or more limited partners, only those owning 10% or more of the limited partnership need to be listed. You may include the information on a separate sheet. List the percentage of ownership held by each limited partner. When one or more General Partner is a legal entity, including a flow chart to show the relationship of the legal entities will help the OLCC to process your application quickly. Server Education Designee x If you are applying for a liquor license that allows customers to drink alcohol at the business you must list a person to take an OLCC-approved Alcohol Server Education class for the corporation.
