Oakland County Circuit Court - Family Court Division, Adoption Services Requesting Passport Assistance An adult adoptee trying to obtain a passport may run into difficulty with the passport office if there is a difference between his or her date of birth and the date the birth certificate was registered with the State. You may need a certified letter from the county in which your adoption took place. To request this letter, please submit the following information to 6th Judicial Circuit Court - Family Division, Adoption Services, Oakland County Service Center, 1200 N. Telegraph Road, Pontiac Michigan, 48341-0452: A written request specifying your need for passport assistance, including the following: a) name b) date of birth c) adoptive parents names at the time of the adoption d) approximate date the adoption occurred e) a copy of your driver's license or picture identification. Requesting a Certified Copy of the Order of Adoption If you are an adult adoptee or the adoptive parents of an adoptee and are trying to obtain a certified copy of the Order of Adoption (final adoption decree), please submit: A written request specifying your need for a certified copy of the Order of Adoption, including the following: a) name b) date of birth c) adoptive parents' names at the time of the adoption d) approximate date the adoption occurred e) $10 fee (check or money orders are made payable to: Oakland County Clerk) Please note: checks are not acceptable from out of state residents. f) a copy of your driver's license or picture information.
