Young If you're requesting information from an adoption record or requesting a Confidential Intermediary search, the Court requires the checked information below: A written request specifying the information you're seeking from the record. The Confidential Intermediary Petition filled out, both front and back. The enclosed forms filled out completely. Request by Adult Adoptee for Identifying Information Release of Information Authorization Adult Adoptee Parent's Consent/Denial to Release Information to Adult Adoptee Other: The appropriate fee. Check or Money Orders are made payable to: Oakland County Clerk. $10 for each certified copy $20 - to send Clearance to Central Registry in Lansing $60 - non-identifying information request $20 - filing fee for the Petition for Confidential Intermediary $250 - the Confidential Intermediary search A copy of your driver's license or picture identification. A copy of your non-identifying information already obtained. Proof of relationship to adoptee (cc of birth certificate to show common parent). Once the above checked information is received and reviewed, an agency or court caseworker will be assigned and you will be contacted.
