American LegalNet, Inc. www.FormsWorkflow.com GOVERNMENT OF THE DISTRICT OF COLUMBIA ALCOHOLIC BEVERAGE REGULATION ADMINISTRATION INSTRUCTIONS FOR FILING AN APPLICATION FOR ALCOHOLIC BEVERAGE CONTROL ENTERTAINMENT ENDORSEMENT The following instructions are intended for retail applicants who are applying for an Alcoholic Beverage Control (ABC) entertainment endorsement. You must call an ABC Licensing Specialist to schedule an appointment for the submission of your application. Applications will only be accepted when ALL of the information is provided. To schedule an appointment call (202) 442-4423 between the hours of 9:00 a.m. and 4:00 p.m., Monday through Friday and ask to speak to a licensing specialist. 1. 2. 3. 4. 5. All persons applying for an ABC Entertainment Endorsement must be at least 21 years of age. Applications must be submitted in person. Please bring a valid government issued identification with you. Please note the term "APPLICANT" as used in this application designates the person(s) in whose name(s) the license will be issued if the application is approved. Application forms must be notarized where applicable. FEE: The application must be accompanied by the proper endorsement fee in the form of a cashier's check, certified check, business check, attorney's check, personal check, or money order, payable to the D.C. Treasurer, cash, or by credit card (except for American Express). The entertainment endorsement fee is twenty percent (20%) of the base license (class C/R, D/R, C/H, D/H, C/T, and D/T) fee. Your licensing specialist will provide you with the correct endorsement fee. Please see the attached fee schedule. Attach extra sheets if the space provided under any item is inadequate. Write, "see attachment" in any such space, and show name of licensee and date of application at the top of each sheet. 6. Instructions for the Entertainment Endorsement Application: Please read all questions carefully. Each question must be answered. If a question of one portion of the question does not apply; fill in the word "NONE". 1. Check the type of activity the applicant is seeking approval for: Entertainment, Dancing, and/or Cover Charge. i. Note: "Entertainment" includes live music or any other performance by an actual person, including live bands, karaoke, comedy shows, poetry readings, and disc jockeys. "Entertainment" does not include the operation of a jukebox, a television, a radio, or other prerecorded music. ii. Note: "Cover Charge" is a fee required by an establishment to be paid by patrons for admission that is not directly applied to the purchase of food or drink. b.
