STATE BAR OF WISCONSIN LIMITED LIABILITY LEGAL PRACTICE Instructions The registration form and annual updates are to be filed with the State Bar of Wisconsin in order for a lawyer or a law firm to be recognized as a Limited Liability Organization under SCR 20:5.7. The registration form must be signed by a lawyer who is licensed to practice law in Wisconsin and who holds an ownership interest in the organization seeking to register under SCR 20:5.7. Registration is on a July 1 through June 30 year. The registration form indicates the date the registration expires. Complete the registration form by providing: The name and address of the limited liability organization being registered. The state or jurisdiction in which organized, if other than Wisconsin. The attorney names, residence addresses, states or jurisdictions where licensed to practice law, and attorney registration numbers of the lawyers in the organization and the nature of their ownership interest (such as partner or shareholder) in the organization. For Wisconsin licensed lawyers, the attorney registration number is the seven digit number appearing on the membership card members receive after paying membership dues. If an attorney is licensed in two or more jurisdictions, please use separate lines to indicate each state or jurisdiction and attorney registration number.
