For Clerk's Use Only SUPERIOR COURT OF ARIZONA MOHAVE COUNTY Case Number: (Name of Petitioner) ORDER FOR PROTECTED ADDRESS AND (Name of Respondent) Upon Request of Petitioner Respondent, and good cause appearing, IT IS ORDERED that: The address of Petitioner further order of this court. The Clerk of the Court and the parties hereto shall comply with the requirements of Rule 7, A.R.C.P. The Clerk of the Court shall promptly mail the documents by regular first-class mail to the most recent protected address provided to the Clerk, and service shall be deemed complete upon mailing. The Clerk of the Court shall promptly file a written statement verifying the documents that were mailed and the date of mailing to the protected address signed by the Clerk of the Court or Deputy Clerk who mailed the documents.
