In the State Court of Chatham County State of Georgia Vs _____________________________________________,Defendant ALTERNATIVE REPORTING Rules for and Petition to Report for Probation via U. S. Mail or Electronic Media Probation is a privilege granted by the Court to selected defendants, conditioned upon compliance with several general and special requirements established by the Court in each case. Personal interaction with the probation officer is the single most important element of the Court's sentence in the above cited matter. The Court recognizes that reporting "in-person" may sometimes be difficult and costly, especially if the defendant resides outside of Chatham or a surrounding county. The Court, may, for good cause shown, authorize alternative reporting in cases with extenuating or special circumstances where the defendant is ALSO in compliance with all other requirements. ALTERNATIVE REPORTING is managed by PRIDE probation. ALL defendants, even those where sentencing orders indicate that alternative reporting MAY be authorized, are required to complete their initial appointment at the offices of PRIDE Integrated Services, Inc., 210 E. 31st Street, Savannah Georgia 31401. There, the defendant's official intake will be completed and the specific requirements of the sentence will be reviewed including ALL financial obligations and the special and general conditions of probation. At any time after satisfying all initial intake requirements, a defendants may seek approval for ALTERNATIVE REPORTING through his/her probation officer. I, the defendant cited above, request permission to report to my probation officer at other than the offices of PRIDE Integrated Services, Inc. In my sentencing order, the Court indicated that such reporting may be authorized. My initial intake has now been completed. I understand and agree to comply with all requirements of my sentence and the alternative reporting process. . I hereby petition the Court to allow me to report to my probation officer from __________________________ by use of ____ US Mail or _____the INTERNET, http://www.E-Probation.org beginning ______________ because: On my next probation appointment I will appear at PRIDE, in-person and receive specific instructions regarding PRIDE's alternative reporting requirements. I consent to all such requirements including each of the following: 1. I will comply with all standard and specific Conditions of Probation and provide proof of such compliance as required by my Probation Officer. Failure to do so may result in my being required to report in person and may be considered a violation of my probation. That would subject me to a warrant being filed for my arrest. 2. I will not change my address (physical or e-mail) or phone number throughout the period of my probation without written permission from my probation officer. I recognize that any action on my part that interferes with my probation officer's ability to contact me may be considered a violation of my probation and subject me to a revocation of probation charge and a warrant for my arrest being sought. 3. If advised by my probation officer that a violation of probation has been filed and a warrant for my arrest has been issued, I will, surrender myself to the Sheriff of Chatham County within fifteen (15) days of such notification. 4. I hereby authorize and consent to the release of all information regarding my probation. 5. I agree to comply with the following specific requirements, if any: I completely understand all requirements indicated above and the consequences I face for failure to comply. I also understand that if I do not adheare to the alternative reporting requirements I will be considered to have absconded from probation and will be treated accordingly.
