The Department of Children and Family Services has not ensured "that all parties to the permanency hearing (were) provided a copy of the most recent service plan prepared within the prior six months at least 14 days in advance of (this) hearing" as required by Section 2-28 of the Juvenile Court Act; and/or b. An Administrative Case Review has not been held within the past six (6) months, as required by policy and/or procedures.
