STATE OF ILLINOIS COUNTY OF MCLEAN ) )SS ) IN THE CIRCUIT COURT OF THE NINETEENTH JUDICIAL CIRCUIT, MCLEAN COUNTY, ILLINOIS IN THE MATTER OF _________________________________ BY PARENT OR GUARDIAN FOR CHANGE OF NAME ) ) ) ) ) CASE #_______________________ DECREE (MINOR) This cause having come to be heard upon the Verified Petition filed herein, and the Court having considered said Petition; and being fully advised in the premises, doth find: That all material facts alleged in said Petition have been proved sufficiently; that said Petitioner is now and has been a resident of the State of Illinois for six months or more prior to the time of filing the Petition herein; that the conditions mentioned and specified in 735 ILCS 5/21 have been complied with; that this Court has jurisdiction of the Petitioner, has considered the relevant factors as specified by statute, and finds that the prayer in the Petition should be granted and that said change of name is necessary to serve the best interest of the minor Petitioner.
